The anatomy of the region of the iliopsoas is shown in Figure 4 . The iliopsoas bursa is the largest bursa in the body and communicates with the hip joint in 15 percent of patients. 4 , 26 Bursitis is caused by overuse and friction as the tendon rides over the iliopectineal eminence of the pubis. The condition is associated with sports requiring extensive use of the hip flexors (., soccer, ballet, uphill running, hurdling, jumping). Iliopsoas bursitis is characterized by deep groin pain, sometimes radiating to the anterior hip or thigh, often accompanied by a snapping sensation. (See section on snapping hip syndrome.) If severe enough, it may be accompanied by a limp. 8

I'll use the right leg for our example, as illustrated in the picture [on page 48]. The patient is supine, with the right leg in the same 90-90 position I described above. I am sitting on the patient's right side, with my right shoulder against their posterior thigh. I use both of my hands to grasp firmly around the whole of the upper thigh. My left hand will be the main hand, with the heel of my left hand pushing against the greater trochanter. I pre-tension the area by both lifting the whole thigh superior and taking it into internal rotation to the feather edge of the barrier. I then do a quick thrust into further internal rotation. I really don't use much fine-tuning of three dimensions here; it's a pure thrust into internal rotation.

This posture can also distort the lower back (lumbar spine) from it’s normal curve. It is meant to stack up in a curve, and not curve backward. With that reverse curve position, the front edges of the vertebrae press together and can squeeze disc material out toward the back of the spine where the nerves are located. If the pressure on the nerves is sufficient or the discs are leaking fluids containing irritating chemicals, it can hurt. The position also affects the muscles, compressing some and stretching others. Constant poor positioning can cause the muscles to contract and even go into spasm.

Two other things we’re going to talk about real quick to help with stabilizing the lower back, and therefore piriformis issues and the pelvic issues as well as what one might perceive as sciatic type pain are the psoas muscles which do this, bring your leg up and out and flex it, and again on the left. This would give you more pain doing that from left to right, or just bending forward as if you were touching your toes. A lot of times if someone’s feeling, even though you might feel back pain as you go and do that, it’s actually coming from the front.

I have some info.  I developed this same ileopsoas tendinitis two weeks after left hip and two weeks after right hip replacement.  The surgeon denied it could be the hip causing this.  He sent me to a rheumatologist and low back surgeon, wasting my money.  He is considered the best surgeon in the state!  He got so frustrated with me.  The pain became so severe I could barely walk.  I was in misery.  Finally, he put cortisone in the trochanter bursa and the pain went away in a couple days.  When the same pain occurred after the second hip, I got the cortisone.  I have also tried cortisone directly into, by ultrasound, the tendon.  That did not work any better than just doing the bursa.  So every three months for five times each hip, I got cortisone.  I went to PT and stretching made the pain severe and debilitating.  I also had several dry needling from the PT which did nothing.
